Unlocking additional equipment slots means making the most of the Mission system. Accessible via any save point in the game, you're free to ignore almost any of them - but plenty of the Crisis Core missions hide some incredibly helpful rewards.
But if you'd rather not breeze through the 100+ missions looking for the equipment slot increases, here are the two you'll want to seek out:
- M7-2-2: Search and Destroy II
- M7-2-5: Operation Mako Reactor III
Completing Mission 7-2-2: Search and Destroy II rewards the Keychain, allowing you to equip another accessory for a total of three.
Completing Mission 7-2-5: Operation Mako Reactor III unlocks the Backpack, increasing your accessory slots to four.

To unlock each of these missions, you just need to complete the one before it. The first of these missions unlocks during Chapter 4. Once you're at that point, you can push through them in order to max out your accessory slots.
Just be warned that they get harder and harder, capping out at five-star rank for the Backpack mission. You may have to come back further into the game to successfully beat it.
